It's a reference to Wisconsin poker player "Big Al" Emerson.

2. Katie

It's an imitation of K-T

3. Ken

It's a portmanteau of "King" and "Ten". Portmanteau is method of word formation, a variation of this method, as the "reduction".

4. Ike Turner

Beats QT. QT has nickname Tina Turner. Ike and Tina Turner were married. As it later turned out, Ike Turner beat and raped his wife.

5. Woodcutter (offsuit)

Possibly from the legend of King Edward and the Woodcutter
